Most people are cautious about starting a new medication. They read the label, ask about side effects, maybe look it up online before taking the first dose. But stopping a medication, particularly one that’s been on the list for years, often happens without the same level of consideration. It runs out and the refill doesn’t get picked up. The side effects become intolerable and the person decides to stop. A new physician doesn’t agree with a previous prescription and discontinues it abruptly.
What most patients don’t realize is that stopping certain medications suddenly can be as medically significant, sometimes more so, than starting them. For some drug classes, abrupt discontinuation produces withdrawal syndromes, rebound effects, or physiological consequences that are serious, occasionally dangerous, and entirely preventable with appropriate tapering and medical supervision.
Why the Body Adapts to Medications Over Time
When a medication is taken consistently, the body doesn’t remain passive. It adapts. Receptors that are repeatedly stimulated or blocked by a drug change in number or sensitivity to compensate, a process called physiological dependence that occurs with many medication classes entirely independent of whether the drug is addictive in the psychological sense.
When the drug is suddenly removed, those adaptations are exposed. The systems that had adjusted to the drug’s presence now have to readjust to its absence, and that readjustment takes time. In the interim, the body can be in a state of significant physiological imbalance, producing symptoms that range from uncomfortable to medically dangerous depending on the drug and the patient.
The Drug Classes That Carry the Highest Risk
Benzodiazepines and sedative-hypnotics. This is arguably the most dangerous class to stop abruptly. Benzodiazepines, diazepam, lorazepam, alprazolam, clonazepam, and related sleep medications work by enhancing GABA, the brain’s primary inhibitory neurotransmitter. The brain adapts by reducing its own GABA activity. When the drug is removed suddenly, the result is a state of CNS excitation that can produce anxiety, tremors, insomnia, and in severe cases, seizures and delirium, outcomes that can be life-threatening. This is not a rare complication of abrupt benzodiazepine withdrawal in long-term users; it’s a predictable physiological consequence of how the brain adapts to these drugs.
Antidepressants. Abruptly stopping antidepressants, particularly SSRIs and SNRIs like sertraline, venlafaxine, and paroxetine, produces what’s called discontinuation syndrome. Symptoms include dizziness, electric shock sensations, nausea, irritability, anxiety, and vivid dreams. While not medically dangerous in the way benzodiazepine withdrawal can be, it’s highly unpleasant and frequently misinterpreted as relapse of the underlying depression, which leads to restarting the medication when a gradual taper would have resolved the symptoms. The question of how sertraline affects cognition and wellbeing is part of understanding why these medications require careful management both when starting and when stopping.
Beta-blockers. Stopping beta-blockers abruptly, particularly in patients with coronary artery disease, can produce rebound tachycardia, elevated blood pressure, and in some patients, a significant increase in the risk of angina or heart attack. The heart has adapted to the lower workload the beta-blocker enforced; when the drug is removed suddenly, the adrenergic system rebounds, sometimes dramatically. Patients who stop their beta-blocker because of side effects or a missed refill without telling their physician are taking a risk they may not be aware of.
Blood pressure medications. Certain antihypertensive drugs, clonidine in particular, produce severe rebound hypertension when stopped abruptly. Blood pressure can spike dramatically within 24 to 48 hours of discontinuation, creating real cardiovascular risk. Even less dramatically, stopping other antihypertensives abruptly can allow blood pressure to rise quickly in patients whose hypertension has been well controlled.
Corticosteroids. Long-term use of prednisone and similar steroids suppresses the adrenal glands’ own cortisol production. When steroids are stopped abruptly after prolonged use, the adrenal glands may not be able to produce adequate cortisol quickly enough, a condition called adrenal insufficiency that can produce fatigue, nausea, low blood pressure, and in severe cases, an adrenal crisis that is a medical emergency.
Anticonvulsants. Stopping seizure medications abruptly is a significant risk for patients with epilepsy, where uncontrolled seizures are a direct and serious consequence. Even in patients taking anticonvulsants for other reasons, nerve pain, mood stabilization, abrupt discontinuation can produce withdrawal effects and seizure risk in people who had no prior history of seizures.
Opioids. While opioid withdrawal is rarely life-threatening in otherwise healthy adults, it is profoundly uncomfortable and often what drives people to resume use. In older adults or those with cardiovascular disease, the physiological stress of opioid withdrawal can be more consequential. More importantly, patients who stop opioids abruptly and then resume them have lost their tolerance, meaning that a dose that was once manageable can now be dangerous.
Why This Matters for Older Adults Especially
The risks of abrupt medication discontinuation are amplified in older adults for reasons that should be familiar from any discussion of medication management in this population. Multiple medications mean multiple potential withdrawal risks. Reduced physiological reserve means the body is less able to compensate for the disruptions that abrupt discontinuation produces. Cognitive impairment may mean a patient doesn’t recognize or report withdrawal symptoms accurately. And the presence of cardiovascular disease means that rebound hypertension or tachycardia carries more risk than it would in a younger, healthier patient.
This is one of the reasons that medication review in older adults has to account not just for what should be added or what’s interacting, but for what’s on the list that would need to be carefully managed if it were ever to be stopped. A physician planning a deprescribing conversation, removing medications that are no longer necessary, has to include a plan for tapering, not just stopping.
What to Do Instead
The principle is straightforward: never stop a medication that has been taken for more than a few weeks without discussing it with your physician first. This is true even if the medication is causing side effects, even if you’ve run out of refills and can’t get them quickly, and even if you’ve read something suggesting the medication isn’t appropriate for you.
A physician can determine whether abrupt discontinuation is safe for a specific medication, prescribe an appropriate taper schedule, monitor for withdrawal effects, and manage rebound symptoms if they occur. What they can’t do is prevent complications from a discontinuation that happened without their knowledge.
