3. Appointment Requests and Communications

Requests submitted through the Site (such as appointment or contact forms) are not confirmed until our office responds. Do not use Site forms, email, or text messaging to send urgent or emergency medical information. Standard email and SMS are not fully secure channels.

4. Messaging Terms (SMS/Text)

By providing your mobile number and opting in, you consent to receive text (SMS) messages from Prime MD Plus, which may be sent using automated technology and may include appointment reminders and confirmations, review or feedback requests, billing notifications, and other care-related communications. Message frequency varies. Message and data rates may apply.

• Carriers: Wireless carriers are not liable for delayed or undelivered messages.
