How Pain Presents Differently in Older Adults

Pain is one of the most common reasons people seek medical care, and one of the most undertreated conditions in older adults. Those two facts coexist because pain in older adults frequently doesn’t present the way it does in younger people and when the presentation doesn’t match expectations, the pain often goes unrecognized, minimized, or attributed to aging rather than addressed as a clinical problem that deserves proper evaluation and management.

Understanding why pain presents differently with age, and what that difference means in practice, is useful for older adults, for their families, and for any physician who wants to provide genuinely comprehensive care to this population.

Why the Experience of Pain Changes With Age

The biological processes that govern pain perception change significantly as the nervous system ages. Several of those changes work in opposite directions simultaneously, which is part of what makes pain in older adults clinically complex.

The pain threshold, the point at which a stimulus begins to register as painful, tends to increase with age. Older adults may not feel mild to moderate pain as acutely as younger people would. This sounds like a benefit, but it’s not. It means that a condition producing mild pain in a younger person may produce no pain at all in an older adult, allowing serious pathology to develop further before it becomes symptomatic.

At the same time, pain tolerance, the ability to endure ongoing pain, tends to decrease with age. Once pain is present and chronic, older adults often find it more difficult to cope with than younger adults experiencing the same objective intensity. The relationship between chronic pain, depression, and functional decline is tighter in older adults than in younger ones, partly because the neural resources for emotional regulation and pain modulation become less robust with age.

The autonomic nervous system, which contributes to the physiological expression of pain, the racing heart, the elevated blood pressure, the sweating that accompanies acute pain in younger people, becomes less reactive with age. This means that the objective signs that alert clinicians to pain are less reliably present in older patients, making assessment more dependent on careful history-taking and observation.

How It Shows Up Differently in Practice

The most clinically significant difference between pain in older and younger adults is the atypical presentation of serious conditions.

Heart attacks are the most documented example. Classic myocardial infarction in younger adults typically produces crushing chest pain radiating to the left arm or jaw. In older adults, particularly women and diabetics, the same event may present as fatigue, shortness of breath, nausea, confusion, or vague discomfort that doesn’t feel urgent. Many older adults who have had heart attacks report that they didn’t feel what they expected a heart attack to feel like, which delayed their seeking care. This atypical presentation has real mortality consequences.

Abdominal emergencies follow a similar pattern. Appendicitis, perforated ulcer, bowel obstruction, and mesenteric ischemia can all present with less dramatic pain in older adults than in younger ones, sometimes with only mild discomfort or no abdominal pain at all. The conditions progress while the absent or minimal pain fails to prompt medical attention.

Infections are another area where pain signals are muted. A urinary tract infection in a younger adult produces the familiar burning discomfort that drives them to seek treatment. In an older adult, the same infection may produce no urinary symptoms, instead manifesting as confusion, behavioral change, or functional decline. Chronic Pain Is Different Too

Beyond acute presentations, chronic pain in older adults has features that distinguish it from chronic pain in younger populations.

Older adults are more likely to underreport chronic pain. This is partly cultural, a generation that was taught to manage discomfort without complaint. It’s partly cognitive, people with early dementia may lack the language or insight to accurately describe pain and instead express it through behavioral changes: agitation, resistance to care, withdrawal, changes in sleep or appetite. And it’s partly the result of having normalized chronic discomfort as a feature of aging rather than as a symptom warranting attention.

The consequences of undertreated chronic pain are significant. Pain that goes unmanaged drives depression, disrupts sleep, reduces physical activity, accelerates functional decline, and increases fall risk. In older adults, each of these consequences compounds the others in a cycle that’s difficult to break without addressing the underlying pain directly. Medical conditions that increase fall risk consistently include undertreated pain, both because pain limits movement and because the medications used to manage it improperly can affect balance and coordination.

The Complexity of Pain Management in Older Adults

Treating pain in older adults requires navigating a set of constraints that doesn’t apply in younger patients.

NSAIDs, often the first-line approach to musculoskeletal pain, carry elevated risks in older adults: gastrointestinal bleeding, kidney injury, fluid retention, and worsening of heart failure and hypertension. Opioids, when used, require careful dose adjustment because of altered pharmacokinetics and increased sensitivity to sedating effects. Certain adjuvant medications used for neuropathic pain, gabapentin, pregabalin, tricyclic antidepressants, produce sedation and cognitive effects in older adults that limit their use or require careful titration.

This is exactly the kind of complexity that requires a physician who sees the full picture, who knows what conditions are present, what other medications are already on board, and how the patient’s physiology has changed. Comprehensive medication management in the context of pain means not just choosing the right analgesic but understanding how it interacts with everything else the patient is taking and managing.

Non-pharmacological approaches, physical therapy, gentle exercise, cognitive behavioral therapy for chronic pain, heat and cold, and targeted interventional procedures, often play a larger role in pain management for older adults precisely because the pharmacological options carry more risk. These approaches require time and coordination to implement, which is another reason why continuity in a geriatric care relationship matters for this population.

Pain Is Not a Normal Part of Aging

The most important clinical message about pain in older adults is this: pain is not something to be expected and endured as a feature of getting older. It is a symptom that warrants evaluation, a diagnosis that warrants treatment, and a quality of life issue that deserves the same clinical attention as any other condition.

When an older adult says they’re in pain, they should be believed and when they don’t say it but something else has changed, pain should be on the list of explanations to consider.
